Pocket revision

Dr. Hueneke,

I am, for the most part, very happy with the results of my breast augmentation (performed 6-07; 375cc saline mod unders) and will not elect to have another surgery any time soon. However, during surgery my creases were lowered quite a bit; my breasts now have no actual "crease" and even have a slight bottomed-out appearance, with the nipples appearing high on the breasts. They look fine in clothes and even nude when my arms are at my sides, but in other positions they appear very unnatural.

Again, this is not bothersome enough for me to undergo a second surgery any time soon, but when the time comes for me to consider replacing my implants I would like to have the creases revised in order to create a more natural and aesthetically pleasing appearance.

My questions are: How difficult is it to revise pockets?
Do such revisions usually "take" or will I risk a high chance of the new creases failing?
Do surgeons typically charge more for a pocket revision than for a breast augmentation without a revision?

Technically it is easy to fix the pocket. The hard part is getting it where you want and it staying there. The rate of success can vary. It is more costly to do this than an implant exchange alone.

Thank you for your response. When you say "staying there," do you mean that there is a good chance that the placement will fail? And if so, can this happen over time, even once a revision has healed?

Hi

I know you're awaiting a reply from Doc. Hueneke, but I just wanted to let you know I've got the "no crease" look too. I underwent breast augmentation with crease incisions, which are riding high. My nipples aren't, but they are assymetrical if that makes you feel any better.

Also, no matter what position my breasts are in, they look unnatural, at least to me, at this point. Due to the fact I have no natural breast tissue, I do not believe they will change much. The implants just do not blend in.

However, overall I am pleased that I can wear low-cut shirts and can fill out a bra. I cannot wait to go to the beach. I've never owned a bathing suit in my life and have never went to the beach (well, that's a lie. I went when I was a baby and at age 9, in a one-piece ; )

At least for my practice there a good chance it will stay where I put. I try to over-correct a little. What I meant to say was it is not predictable how well everything stays in place. Once a couple of months pass, then usually things do not change much.
