Rippling & Lifecell

Hi Dr. Hueneke,

I had my breast augmentation November 2008; had a hematoma & went back to surgery 5 days PO. I have 322 textured overs.

About 3 months ago, I started noticing rippling on the insides of both breasts. It has continuously gotten worse since then. Now the ripples are very evident on the insides & outsides of both sides.

I saw my PS a month ago & she did view the rippling. I went back to see her last week & she said the best course of action to take at this point would be to place Lifecell ADM inside the pockets on the insides of my breasts. Going back in to put them under the muscle doesn't really guarantee that the rippling won't come back. We wouldn't do anything to the outsides since they are unable to be seen by anyone but me.

Do you have any experience using this or similar brands like Alloderm? And for a situation like mine? I keep reading about Alloderm being used for breast reconstruction, but not for issues like rippling. What is your opinion on this?

Money is not the only issue here. Going submuscular is a good way to decrease rippling. Alloderm is another way. I have had no patients of my own that I am aware of that have medial rippling. Therefore I have not had a need for alloderm. I have used it for this application in reconstructive patients and it does help. I have to question the wisdom in over the muscle textured implants. I will say even the thickest alloderm is thinner than the pectoralis muscle in most people.

My PS never asked if I wanted textured - she just said it was best & I trust her, so I never questioned it. We also did not have much discussion about having overs or unders. I did initially express a desire for overs due to the lesser pain & recovery period, which she fully agreed was best for the amount of breast tissue & my build, as well as for my lifestyle.

My PS has offered to do the Alloderm surgery for free & to also use the company's complimentary samples on me. Therefore, I would only be paying the hospital & anesthesia fees. Honestly, that will still be more than we can afford right now, but the rippling is really bothering me & taking quite a bit of my happiness away from what should be a very happy thing.

Dr. Hueneke, in your opinion, if my rippling is only getting worse with time, is there much chance it can recover on it's own w/out some type of surgery? And if not, should I revisit the question of doing unders instead of Alloderm w/ my PS? Is there really a lower ratio of rippling w/ textured silicone unders?

Is Alloderm filler permanent? I would make sure it is not something that has to be touched up every few years; otherwise, over the long haul, it seems like a pocket change to smooth might produce the best results and save money over time. I don't know much about Alloderm. Good luck!
