Cleavage Question

I hope you can help since my PS left me even more confused...
I had a BL/breast augmentation in 2007 with 510cc mod saline unders. They were way too big 34E/F, I never had cleavage and after they dropped I had no upper pole fullness either. Apparently my skin stretched with the weight of the implants as well. Had a redo Feb 2008 with a re-lift and 390cc mod saline unders. I am much happier with the size. They look smaller from the front (I think because they go out to the sides), but they are DD's. I am unhappy with the shape, the lack of cleavage and upper pole fullness. I know my natural breasts grew outwards too and that has a lot to do with it, but does that mean I will never have what I am looking for? I would like a rounder breast with a full upper pole and decent cleavage. I just talked to my PS about a revision, wondering if he could tighten the pocket on the outside and bottom (right now my breasts fall into my armpits when I lie down). He doesn't think that is the problem and wants to relift (again!) and swap my 390's for 450's. How will this help when I had 510's and still no cleavage? And how is it that I would need a 3rd lift in less than 2 years? Did he not lift enough to begin with? Or is it really my body? He also suggested going over the muscle to get them closer. I don't know that I am comfortable with that either. If it helps I am 5'5" with a 31.5" ribcage. My pre-breast augmentation breasts were deflated C's. I have included pics of pre, post 1st time and now. Sorry so lengthy, I appreciate your time and any insight you may be able to provide. Thank you so much!

I could not tell that you had a lift with the 510's. The problem is twofold. Your tissue appears to not be adequate for that size implant. The pockets need to be closed. If you had anything less than an anchor-shaped lift, it was likely not enough. With smaller implants there is less weight, so that seems to be a good decision. Closing the pocket without going larger might be best. Hard to say, but the implants may never have been close enough together. You may not get what you are wanting, but I would suggest seeing another PS.

Thank you for your response! I have just seen another PS who recommends closing the outside pockets and loosening my pec muscles at the top because they are pushing my implants outward. He said if he does another lift, my skin will stretch again. All of this sounds right to me. But then he recommended 350 HP's filled to 450. (I have 390 MP now). I'm worried this will be too narrow but he is promising the cleavage and upper pole fullness that I want. He is the most experienced PS in this area, so I did schedule with him. Does this sound right to you? Thanks again!

There is not a big difference in the dimensions of these 2 implants. The plan seems good, but without seeing you I could say for sure.
