My Revision Surgery

Hello Dr. Hueneke,

Here is my issue. I had my BA in August 2007. In June 2008, I started to notice some assymetry in my breasts - my left breast crease was about a 1/2-1inch lower than my right breast crease. My "higher" breast (right) was also slightly firmer than that leftie. I was unhappy with the assymetry, as I could see it in my non padded bathing suits and also in clothes that I couldn't where a bra with. So, I decided to go for a revision to have this fixed. The doctor told me it would be an easy 10 minute fix, where he would go in and "score" the tissue under my right (he called it a capsular release) to separate the scar tissue and it should help to correct the assymetry. This revision was almost 2 weeks ago on July 18th. Did I have CC?

Well, I'm almost 2 weeks post op and I'm concerned about a couple of things. First of all, I've noticed that my scar is hard and bumpy and I have 3 mondor's cords. The assymetry is still there (not as bad as before) and the right breast is still more firm than the right. Does this mean I have CC forming again???? Should my scar feel like this? Should my breast be slightly firmer than the other one? Or should I just wait it out?

Well, if the breasts looked fine initially and then changed in the time frame you described, I would sat you had a CC. The treatment seems appropriate and would have been my first choice. Although I do not recommend massage, in cases where CC has occurred and I have done a capsulotomy (capsule release), I do in fact have my patients massage. All of the things you are experiencing seem normal and it is too early for CC to be seen on exam. The process if it does occur again starts at day 0 (July 18th). It usually takes a couple of months to see it. I would contact your surgeon to make sure everything is processing properly.

Thank you very much for taking the time to read my post and respond - I really appreciate it.

I just wanted to follow up on this. From your experience, after you have done a capsulotomy, is it normal for the boob to still be higher than the other? I don't know if I'm going crazy or what, but I feel like right after the surgery it looked a little better, but now I feel like the more I look at it (and maybe I'm obsessing) that it just looks like it's back up right to where it was before the surgery. It has taken on a sorta funny shape at the bottom too...will it drop down or is this pretty much going to be my result? I initially thought it was better looking, but now I feel like nothing has changed much...sigh. I will call my surgeon on Monday, but I would appreciate your opinion too. Thanks!!!

It is hard to say without seeing you. Releasing the capsule is very simple to do but very difficult to get the way you want. Since there is variation in healing, trying to predict how things will turn out is hard. I think the goal should be to improve the breast and get it soft. From a position standpoint trying to get it completely even is not realistic. If the breasts are still significantly uneven in a couple months or you develop CC again, I would repeat the capsulotomy.

Thank you, Dr. Hueneke for taking the time to answer my questions! You have pretty much told me a lot of the same things my P.S. has said. It's good to have a second opinion from another good doctor, even though you can't see me. I think I'm just going to wait it out and see what my P.S. says at my next follow up in a couple of weeks. I have been feeling really uneasy about the appearance of my breasts and was wondering if you could take a look at my photos below and tell me if you think this can be fixed...do you think I have bottomed out on my leftie or is there something wrong with my rightie? HELP! I'm so sad...

Well, it's really hard to tell without seeing a preop or an exam. I would think if the left was bottoming out, the nipple position woud be higher. Usually that is the case and I do not see that on your photo. It seems like the issue still my be the right is too tight and high. The weird thing is either breast by itself does not look bad. It is just you are so uneven. I would talk to your surgeon. It appears the right is still needing more work. Sorry you're sad. It is most likely fixable. Hang in there.
