Topamax and Trazodone

I take 200mg of Topamax and 50mg of Trazodone daily for migraines. My PS is about 6 hours away and I've been communicating by e-mail with his office staff. I had e-mailed and asked if I could continue to take Topamax and Trazodone up to my surgery and if by chance I had a migraine if it was safe to take Imitrex the two weeks before (not knowing if it had aspirin, but now I know it doesn't). The gal in the office wrote back and told me she wasn't a pharmacist and that I needed to call a pharmacist and ask if Topamax and Trazadone had aspirin in them and if not I was okay (that wasn't my question about those two drugs - the aspirin question was related to the Imitrex). Rude, I know. My husband said I should have written back and said that was a question for the doctor not for her because he doesn't want to drive all the way out there and have him tell me he can't do the surgery b/c I've been on that medication. Do you happen to know if either of those are harmful prior to surgery?

Topamax

I was cleared by both my PS and my anesthesiologist regarding my daily dosage of 50mg of Topamax, but I decided not to take it the night before my surgery -- just in case. Make sure your PS knows about the medication because you want to be safe if you are undergoing general anesthesia.

I don't know about your preventatives, but I was cleared to use any triptan for my migraines (I use Relpax) ... but of course you need to speak to your PS directly about this. I would insist you speak with him.

I got back to her and told her that I wanted the doctor to answer the question about whether or not I could take the drugs and she got back to me and said that he said both were safe as long as I don't take them the morning of and possibly not while I'm on pain meds. But I will talk with him more about that when I meet with him. I will also talk with my neurologist about that, as I have an appointment with him a few days before my BA.

I would have to look up those drugs to be sure but off the top of my head I would say you can take them - however I am not your PS who may have a different list of medications to be avoided based upon their experience.

Given your communication with the office I would say you need to find another PS. The staff are hired by the PS and represent him/her so if you are having some communication issues now you are bound to have bigger ones later. I am available for my patients as much pre operatively as I am post operatively. If you cannot reach your PS to get an answer to a simple question pre op then you are going to have trouble post op after they have your money.

If you proceed you may be fine, but if you have some problems I can only say you overlooked a red flag.
