Hi Dr Kearney, First consult was yesterday with PS in NC. He said due to 22 and 24 cm (although nipples are above crease) if I want unders I'd need a full anchor lift. I'd like to avoid lift/scars. He recommended overs then with 1/3 under the muscle to help reduce sagging longer. He said all breasts sag over time, under or over but over end up sagging faster. Have you ever done 1/3 under? Your advice, thoughts on this are appreciated. If you need larger ones please let me know thank you. IF your nipples are above the crease it is doubtful you need a full anchor lift no matter where you put the implant. The purpose of a lift is to lift the nipple above the fold. If you have a lot of breast tissue hanging over the fold you may be able to do a concentric lift to take up that excess and move your nipple slightly up. I am not sure what your PS means by 1/3 under. IF if means dividing the muscle attachments and disconnecting the muscle from the sternum or chest bone you don't want to do that. In any event there is no such thing as completely under the muscle, we always divide the muscle attachment near the fold under your breast.
