Hello:
I'm a 31 y/o female. I had BA 8 years ago using mentor moderate 200, filled to 210R/220L. Before surgery I was 32A. I was never happy with results, mostly due to no cleavage ( I do have a wide sternum). Recently I lost a lot of weight, so now I have even less boobs. I want to do a redo to improve cleavage mostly. I've talked with a really respectable plastic surgeon who told me about dissecting medially to improve cleavage. I'm ok with that. My concern is the type of implant he uses. My BWD is 15 cm, I'm 5'3'1/2, 100 lbs. The implant that I currently have has a diameter of 11 cm. He recomended moderate + 350....the diameter on this one is 12.1 cm...which I think its still too short to fill my BWD. Apparenly, he has seen better results with this type of implant in his patients. I just think that having a wide space makes me not an usual patient, and considering other implant model would be better. My question is: What implant size would be better for me to provide best cleavage , in both the moderate and moderate plus, but without being more than a small c ? Which one would look better on me, moderate or mod +? Since I've seen same implant model and size providing different results ,in terms of cup size, in different women, could a 425 mod + look on me like a small c ? |

Looking at your dimensions, I would be suprised if your BWD is 15cm given that you are 5'3 and 100 lbs. However, that being said, I would be cautious when someone starts talking about dissecting medially to improve cleavage. If you have a wide space between your breasts due to your anatomy, it is very difficult to greatly decrease this medially without increasing the risk of developing symmastia over time. This is something to discuss with your plastic surgeon.
In terms of implants, it is hard to say which will work better without actually examining you and getting a good feel on where you are trying to get to. The 425cc would most likely put you in the D range, not a C.

Thank you Dr. Pelletiere....If I you were my PS, what would you suggest for someone as thin as me, and with such a wide space between breast ( 4 fingers at least), that would give the best overall appearance.. I don't like the fake look and my biggest concern is to create clevage, also I have mild ptosis ( clavicle to nipple change from 17 cm to 19 cm)..... I know you need to examine someone to really say this, but generally with your experience.......350 mod or 350 mod +?.......Thank you so much

The moderate plus implants will give you more projection, but the moderate will take up more space both medially and laterally. It is tough when you have a wide sternal space. There is not a good way to decrease this space without weakening the tissues and running the risk of having the implants move to medially. Generally, I prefer the moderate plus implants for their projection, but if you are really 15cm BWD, a moderate would fill out the natural space better.
