Hello Dr. Revis, I have been searching for the right plastic surgeon to fix my problem and maybe combind a couple of procedures that I have been thinking about for a long time, but I am not getting any good answers and I am starting to get discouraged....I hope that you can help me.... The first problem that I have is... in june of 2006 I had 460 HP saliene implants put over the muscle left filled to 475 and right filled to 600cc's...I was severly assymetrical, after I had the augmentation done, i looked very symmetrical and I was finally looking great in clothes and bras....one month later my husband and I did not plan this, but we were pregnant. After the baby was born i breastfed for a month and stopped, once everything settled i noticed my right breast was hard and high up and my left was soft and a bit saggy, so at this point they are pretty uneven again.... So I went for my first consult with my PS and he said to keep them over do a capsuleotomy, replace with 700 and 800cc silicones....I was not convinced becuase of the sagginess in the left breast and chance of C.C again.... I then went on the second consult....he also said same exact thing I went of third consult he said he does not believe in silicone and he would go in and just fill the left to 600cc's and replace my right with 700 saliene and do a capsuleotomy. Definetly not convinced.. I then went on the fourth consult, he gave me the same option as the first two if I didnt want a lift, but he recommended a lolipop lift on my left and a benelli possible lolipop lift on right and some pocket work, possible internal sutures, capsuleotomy and swith to silicone 800cc implants, per my request to go larger...This was convincing, but extremely out of my price range.... so that leaves me back to square one.... I also want to have a rhinoplasty and tummy tuck done, i want to combind my redo with one of the two procedures and leave one for another date can you tell me what you would do if you were my P.S.? and what price range am I looking at, I see alot of your patients are having a lift with silicones done for around $6000 on this wesite... I have read all of your qualifications and I am very impresssed, and I have also seen pictures of women on this site that you have done and I am amazed at your skills... I also saw the specials on your website that say switch your saliene for silicone at $4995 and combination of procedures take an extra $1000 off, if all of these things are true I think I am finally taking a step in the right direction ....thank you for reading, and I hope to hear from you soon.... |

hey ladies this is the conversation dr revis and I had through email's if you want to follow up on this question, thanks............. -------------------------------------------------------------------------------- Good afternoon, The problem is the over the muscle vs. under the muscle pocket with such large implants. Here's what I mean- to switch to under the muscle, which I do a lot, always means that you have to make a small openiong between the fibers of the muscle to get underneath it. This creates a small connection between the over the muscle pocket and the new under the muscle pocket. When you then try to place large silicone implants under the muscle, this opening in the muscle has to be made larger so the implant will fit through it. After the implants are placed under the muscle, you can't close the opening because the muscle is now stretched and the implants prevent you from being able to close off the submuscular pocket from the subglandular pocket. Then postoperatively it is possible that the submuscular silicones can pop up through the opening back into the subglandular pocket. This can occur on one or on both sides. With extra large saline implants the problem remains the same. With this large an implant, either silicone or saline, the likelihood of creating and maintaining a good submuscular pocket is difficult if not impossible. I would prefer to see you do the capsulectomy and go back in the same subglandular pocket with a textured saline implant filled to the 950cc range withe the addition of a donut mastopexy. This will give you the extra large perky look. The cost for the lift/capsulectomy/implant exchange would be approximately $6000, $9500 with a combination rhinoplasty, or $11,000 for the breast/tummy tuck consultation. Let me know which combination or procedure you wish to have and we can figure out the exact cost with anesthesia, OR, etc. Thanks for the photos and the email! Do you think if have the donut masteopexy would my scars stretch from having such a big implant and look bad? I think with the proper technique you can make a donut with large implants look very nice and how long do you think that this X-large perky look will last for? do you think that with that kind of lift I would be able to go braless and look pretty even in clothes? I know eventually I may have to go back and downgrade and have the full lift done and go under the muscle with smaller implants, I also know that, the xlarge look does not last forever and in your medical opinion is it really worth it, this is something that I have researched alot and I really want and desire... but I would like to know your thoughts on this and if it is safe? I do personally think that my left larger breast does need more of a lift than the right breast, and I am worried that the donut lift will not be enough for that breast, what do you think? I was informed by doctor # 4 that I need a vertical lift on the left and donut lift on the right, not sure if this is truly needed? Your thinking is correct in that eventually you may need to make a different choice. But for now while you're young, why not have what you want- large breasts without a lot of scars? I suspect the asymmetry of the right vs. left can be taken care of with the implants and the donut. A friend of mine by the name of Lisa Johnson that had the same problem I have now, is a patient of yours she lives in melbourne and you did her redo on the 5th of this month. She had the same amount difference in her breast size that I have, and her previous doctor did a botched reduction. I'm sure you know the whole story but I have to say the vertical lift you did on her with new implants under the muscle looks fantastic and I am amazed at how you fixed her....Her total price for this was $6,000 everything included and I'm hoping that I would be looking at spending around the same amount for my redo? Im still thinking about combining my procedures, but I am definite about having the re augmentation done.... I think the $6000 price is correct I was also wondering about my C.C. with going back over the muscle....would a textured implant decrease the chances of it coming back and what other precautions could we take to make sure we prevent it from coming back, say it does come back, what would we do from that point, would you have to go back in and surgically remove it, and how much would that cost me? I think by using accolate before and after surgery, performing a complete capsulectomy and switching to a textured implant will dramatically reduce your CC risk- if it comes back then we would immediately start accolate to see if we could reverse it. I am really sorry I am asking so many questions and writing so much, this is just very important to me to find the right plastic surgeon, and alot of them have not taken the time that you have given me already just meeting you, this means alot to me. I do feel like you could potentially be the right surgeon for me and wanting all the other procedures that I want to have done, I would be making a big commitment to a surgeon that I can finally trust for a long time, if that makes sense? Thank you so much for taking your time with me to read my emails and answer me back, I know that you must have an extremely busy schedual, and I do appreciate your valuable time.....I am a very serious patient, and I will have to travel from orlando which is a 4 hour drive there and 4 hours back, I do not mind this if I know I am in the right hands. Mabfl4l - Thank you SO much for posting your conversations with Dr. Revis - I was very curious about the answers as your situation is similar to mine. This has also answered a lot for me, so THANK YOU! Also, WOOHOO Dr. Revis! Haha! You sounds so nice and patient, a big deal considering my first breast augmentation doc barely spent 5 minutes in a room with me before my surgery. Take care!

He really is impressive. I posted a help question on here and had a conversation over email with him about my sagging problem. He's very knowledgeable and VERY honest. I'll be going to see him for my redo when we settle into our new place, no doubt about it!

Hey ladies I just wanted to post my email when I wrote to Dr.Revis's nurse, and i wasnt expecting him to email me back, but he did and it is about going bigger if im not happy with my 950's with a least invasive procedure... this doctor is truley amazing! Good afternoon, Yoly sends her best! I decided to answer your email because of the technical question regarding the implants. Here goes: Its Michelle Sanson again, I have a couple more questions for you if that is ok....I was just wondering how long are my quotes that you sent to me good for? I am trying to get vacation around March, I also have to set up childcare and get my husband to get his vacation at the same time, but this is all in a working progress right now, im just trying to set dates in stone. In general the quotes are good for 90 days, but as long as the hospital O.R. and anesthesiologist do not change their rates then your quote would stay the same. We have to place some sort of time limit on the quote because inevitably the facility will increase their fees at some point in the future. I also spoke to a lady that is going to have her surgery done with Dr.Revis and she told me about the hotels that you recommend under discount if we have to stay after surgery, I would like some infomationa bout that, so I can look into that and check availablility, and prices. Most patients stay at the Courtyard by Marriott. Take a look under our homepage at the link "Information for Out of Town Patients" The last question I have is, Im not sure if Dr.Revis does this kind of procedure, but I am going back and forth about the amount of cc's that I want to get, im not sure I will be happy going 950, but I havent seen what it looks like in my body yet. It could be what I want but, if its not I know i will not be happy, and I would dred having to have another surgery just to fill them more, I have heard of other surgeons that can go back in after surgery and find the valve and fill some more, under local...would Dr.Revis do this type of procedure if im not happy with the size? I know that I wouldent need an implant exchange becuase I will be going for the largest shell anyways, its just a matter of filling it more, if im not completely satisfied, I dont want to go too big and i dont want to go too small, I am a tall girl 5 foot 8 with 39 inch hips and small waist, and I would like for my top to match my bottom.... Don't worry- let's get it right the first time. However, if we don't, it is certainly fairly simple to go back in and add volume to your current implants. I have filled that shell up to 1400cc before. |
