Is it true that once you have implants placed over the muscle, you cannot go to under the muscle placement? I have had 800cc silicone placed over the muscle for 7 years. I sag a little but I am not opting for a lift at this time. I was advised that I cannot ever go to unders now. Thank you, Nikka |

This is not true at all, you can switch from overs to unders, it happens all the time. But from experience with consultations with various plastic surgeons, to go from over to under, they only recommend for me a smaller implant and a lift...this was something that i didnt want(smaller implants heck no!) so I finally found a doctor that i really like and trust, and he said it would be very hard to make a pocket under the muscle when i have had overs at the size i desire to go to which is 800cc shell overfilled to 1100cc's....to go under with that size my implant being so big can push itself out of the new smaller pocket under the muscle and go back into the over mucle pocket, and that is not good....you may want to consider bumping them up to 1050cc's to get that 250cc increase and really see a difference and that will help you more with any kind of moderate sag you may have, if you do...good luck sweetie!

Good evening, Let me clarify my answer- switching overs to unders is a good idea in my opinion and I perform this procedure a lot. This works well for implants of the small to moderate size gels and most salines. However, when switching there is a necessary opening from the over pocket to the under pocket, and this opening needs to be kept quite small so the new implants don't pop back out into the over pocket. Remember when going under the muscle, the muscle will exert a lot of pressure on the implant- if the opening from the under pocket to the over pocket is too large it will squeeze the implant right back through into the over position because the over position has absolutely no pressure placed upon it by the muscle. It's just a matter of the implant moving from a high pressure situation to a low pressure situation. When you use a moderate volume gel or most salines, then this can be done quite successfully. But with a large (800cc) gel implant the opening in the muscle required to fit it in is just too large for this to work. Likewise, while most salines will work in this situation, again when you fill a saline to the 800cc range it will also want to squeeze back through this opening. interesting, not to steal this thread but is 450cc-550cc silicone gel considered a large size or a moderate size? cause I currently have 450cc Overs to which my pockets and skin have stretched out and my implants flip, i have smooth 450ccHP..... would someone go Under the muscle with the range i listed? after having been Over???
