![]() | Over or unders - Saline or silicone
Dr. Walden, I went for 2 consultations. As of now, I think that I am a better candidate for overs being I have sagginess. It is about an inch under my breasts. As far as unders goes for me, if I did get unders, The 1st PS said that I would need a mini lift along with it and the implant put in would have to be smaller being the lift would make my breasts smaller. I also did not want a long recovery being I do have small children. That is why I am thinking as of now that overs is the best choice. The 1st PS also said that if I am going to go with overs that silicone is the best to use. He said that with saline I would hear the water under my breasts. He said that it would not be a natural feel. Would saline be a good choice for overs? The 2nd PS that I went to specializes in Tuba. He said that the saline overs would be fine for me. This PS though rarely does the crease or aereola incisions with silicone. At this point, I am at the point where this is just too confusing. Also everything seems so complicated. With silicone, I am hearing that you need the mris and with saline, I read that they can rupture easily. None of this sounds like I can be at ease afterwords. I am trying to figure out what would be the best choice if I go through with this? Also would you have a good recommendation of a Staten Island PS? It seems I am not having any luck finding somebody. Any feedback would be great. Thanks. Lisa

![]() | Consultations
Hi, It is definitely confusing to sort through all of the info when seeking consultations for a procedure with a lot of choices and tradeoffs involved such as breast augmentation. Here is what I can tell you: I never perform over the muscle augmentation because I feel like I can get more natural results with less palpability issues and lower capsular contracture rates going under the muscle. That is my opinion and preference. I also do not do the TUBA because the implant has to be put in over the muscle and I have not seen the excellent results with women who have undergone breast augmentation with this method rather than using a more direct approach (people come to me for second opinions and revisions after having surgery elsewhere). True: silicone has a more natural feel. False: silicone can rupture easily Saline and silicone are both very safe and approved by the FDA. Both have very low rupture rates in the latest Pre-market approval data presented to the FDA. I have never had any of my patients tell me they can hear the saline swishing around. I'm not sure that should make or break your decision one way or another. I do not know a specific surgeon in Staten Island so unfortunately am unable to help you there. good luck in your search! Dr. Walden, Thanks for your reply. One of the PSs that I went to for a consult mentioned the option of doing an under the muscle breast augmentation on me, but he would have to do a partial lift with it. Without the lift I would get the double bubble he said. With a partial lift though, would that make my nipple area smaller? Also would that affect breast feeding at all? With an under the muscle breast augmentation and a partial breast lift, I was really concerned that the healing would be very long and I do have 2 small children. Is the healing a lot harder for the unders? I just scheduled another consult for late September. Thanks again. Lisa Quote:
