500+ on petite girls- problems with nipples not being centered?

I'm 5'2", 95-98 lbs, pre-breast augmentation AA and now a small C with under 300 CCs. I would like to get into the 500s next time around, but my surgeon told me that when going so big on petite girls with little to no natural breast tissue, the nipples get to pointing outwards and don't sit right on the breast. I had never heard of this before?? Has anyone had this problem/not had this problem/heard of this problem? Thanks!!

Im 5'3 115lbs I have 550 mod + and my nipples sit in a good place. Ive never heard of it either. Maybe bc of a position your nipples are already placed?

I think I might have the nipple issue a little bit. I had no tissue pre breast augmentation, looked like an 11 year old! Right after surgery my nipples looked fine but as I started to drop and fluff they went a little easty westy on me, pointing out to the sides a little. It was more noticable when one had dropped and one hadn't. Now that both have dropped I don't really mind it and I still think they look OK. I have pictures up from when one had dropped and the nipple started to go a little wonkers on me, just send me a friend request.
