I am 20 years old and had breast implants put in 7 months ago. The top half of my right areola is puffy. The bottom part is flat and kind of dipped.

Sometimes it looks normal and other times I look down and it's like the nipple faces downwards because the bottom half of the areola is not puffy. Is this an issue? Is it due to scar tissue?

If the appearance of your nipple and areola varies but at times appears normal it is likely due to recovering sensation which can take up to 2 years after surgery. The changeable contractile nature of the nipple and areola in response to temperature change or stimulation is normal. It would be more of a problem if it always remained abnormal in appearance and could well be due to scar tissue especially if the periareolar incision approach was used.

A periareolar scar for implant insertion may have resulted in a scar contracture at the bottom of the areola. Applying pressure and massage may be beneficial.

Thank you for your question! Implants magnify the way your breasts look, and typically do not affect the nipples at all. Sometimes when putting tissue under tension, it can cause the areolas to appear bigger and not as “shriveled up." It is best to see your surgeon to make sure there isn't any developing scar tissue.

Thank you for your question. Did your plastic surgeon use an areolar incision?

I would recommend following up with your operating surgeon for an evaluation as it is difficult to determine the cause of your areola's appearance based upon description alone. It is possible that if an areolar incision was used that there could be some scar tissue pulling on your nipple, but it would be necessary for you to be evaluated in person for a true diagnosis.
