I am 5'3" 118lbs. Before kids I was a DD/DDD. I really want to go back to that. My PS recommended 450 Harrell's Inspira textured. He said my nipples are perfect and the implants will help to decrease the post-baby sag w/o a lift. I'm worried. After looking at other pics of women with 450, I feel that they will be a lot bigger than I want. Think I will go with the 415. Thoughts? He also said high profile would be good for my narrow frame/prevent side boob. I measured 32 around and 13.25 cm.
The implant size sounds great, but you are truly ptotic, you may still need a breast lift. If you have pseudootosis, you will likely not need a mastopexy.

Without seeing a picture I can't accurately answer your question. There are varying degees of droopiness. Very mild cases (pseudoptosis or grade I ptosis) can be improved with implants; more signigicant degees of ptosis need a lift. A high profile implants can help the result in patients with mild ptosis. I don't think you will see much of a difference between a 450cc implant and a 415cc implant cup, at least size wise. If you don't want to be that big, perhaps you should consider a smaller moderate or low profile implant with a lift.

It really depends on how large you are currently. For about every 120-150cc's, your cup size will increase by one. The difference between a 415cc and 450cc implant is about one-quarter cup size.

If your nipple is above the fold under your breast, you can probably correct it with an implant. If your nipple is at or below the fold, then you might need a lift. If you have a lot of excess skin, you may do better with a lift. Some patients don't want a large implant to take up the excess skin. If you are a person who does not want to be excessively large, then I might point you toward a lift; you will be better in the long run.

Although I cannot see your appearance, I agree that your fears are likely going to come true: you'll be too large and still look saggy. My other concern is based on your description of the interaction with your surgeon: he's pushing implants without a lift. That's a red flag that he is either inadequately trained, not board certified, or just incompetent to perform a lift. Please visit a few ABPS certified/ASAPS member surgeons who specialize in all aspects of breast surgery.

Unfortunately no plastic surgeon is able to accurately predict what cup size you will end up wearing after breast augmentation with a certain size implant. I would suggest that you not base your communication or your satisfaction with the results of surgery on achieving a specific cup size.
I would also suggest that you communicate your goals clearly with your plastic surgeon (I prefer the use of goal pictures) and ask him/her to do his/her best to achieve the results you are looking for. You may want to leave it up to him/her to use a breast implant that he/she feels will do the job based on intraoperative evaluation.
Again, the preoperative communication of goals is critical. Once you feel comfortable that you have communicated your goals clearly (assuming you have chosen your plastic surgeon carefully), you will likely feel comfortable allowing him/her to choose the appropriate breast implants for you.
