My left breast never looked sealed and when the scab came off from under the areola, it left a huge gap and the doctor gave me antibiotics and ointment. It's sealed now but a small spot is weaker feeling and the skin around it looks puckered as it pulls together.
Yes, it almost always smooths out with time, and then can always be revised as necessary.

There are occasionally areas of delayed healing post op. These are sometimes areas of high tension or vascular watersheds (relatively decreased vascular supply). In most cases, these areas will go on to heal with local wound care only. As time passes, contour irregularities and pigment variations will settle. Occasionally, they may need surgical revision. However, scar maturation occurs over a period of 12 months (after which, major changes are not commonly noted). Thus, patience is paramount (at least initially).
