Changing from a moderate to high profile will give you more upper pole fullness, but not as significantly as you probably would like. The difference is that the high profile diameter is narrower by only about 1 or so, and that gives more projection. A 40cc increase in volume will add about 1/4 cup size.

It sounds like the implant that slides out toward your armpit needs to have some of the pocket closed. If your implant is more narrow, then you will potentially have less cleavage. If you want more upper pole fullness, you will need a high profile implant. The end result is big breasts. You will still need a push up bra to get cleavage

Whether a 400 HP device is the best implant for you is hard to say without an examination. However, you are on the right track in switching to a higher projecting implant that will help maintain upper pole fullness while limiting the width. As far as falling into your armpit, that will require internal suturing (some call it an internal bra) to help stabilize the implant. The best thing to do is to go for a few consultations with board-certified plastic surgeons and see what they recommend. I hope this helps.
