I had my first BA in August 2014. I was 5'4 and 115 lbs. My doctor put in 525 cc HP implants. I wanted DD and hated the implants because they were way too big. I had a redo last week to take me down to 300 cc HP (I'm the same weight). I'm a D-cup right now, but my breasts are not as perky. The implants don't sit high up on my collarbone or pop like before. My nipples are lower and I know they will drop more as they heal. I'm concerned that I needed a breast lift (my surgeon said it would be OK).
525 cc to 300 cc is very substantial downsizing. I would not be surprised if the deflation of the skin envelope led to some dropping. Are your nipples and implants sitting above the fold?

Your breast may have stretched out, and so it may be better for your appearance to have a breast lift. Unfortunately, reshaping and lifting your breasts will entail scars.

It is hard to tell exactly what is happening without an examination. Dropping from 525 cc to 300 cc is certainly a SIGNIFICANT reduction in breast volume, which could absolutely impact the upper pole fullness. The issue of a lift really depends on skin laxity, the position of the nipple in relation to your fold, and where you carry your natural breast tissue.
Those are things that can partially be discerned with photos but can only completely be assessed in person.

Hi, I do empathize with your concerns. Going from 525 cc to 300 cc implants is a considerable change. Your new implants are not going to have the height nor the projection of the larger ones. You will need time to adjust to the volume, position, and projection difference. The loose skin will also take time to contract down to the new size.
It will take good three to six months for your body to adjust to the new size. Just remind yourself that you wanted the size decrease. If after at time, your nipples are still lower than you desire, or you are still concerned with the decreased height and loss of fullness of your breasts, then consult with your surgeon regarding revision. You may need to consider a larger implant and/or a breast lift at that time.
For now, give yourself and your body time to heal, and to adjust to the new size. Always, keep your communications open with your surgeon who will guide you during these concerning times.

It is often times difficult to downsize by that much without needing at least a minor lift. That said, it would be impossible to give you an opinion about your situation without evaluating you in person.
You are just one week out from surgery, so it is far too early to tell what your final result will be. If your implants are not sitting higher on your chest now, that will not change. Typically the implants sit high after surgery and then they "drop and pop" over the next few weeks. Dropping means they settle into the lower portion of breast, popping means they take on a more natural shape. The good news is that if a lift of some type is ultimately needed, your surgeon may be able to do this under local anesthesia in the office.
