I currently have 700 cc's but am going up to 1050 cc's UHP. They will be placed on top of the muscle. I don't have any sagging in my right breast, but I do have some in my left. With going up this much, what should I expect? How is the sagging going to be? Should I have the implants placed under the muscle?
That's a very significant increase in breast size and can definitely contribute to sagging due to stretching of soft tissues. Sagging can occur whether above or below the muscle.

This is a massive implant, each weighing greater than 2 lbs. With time, you can expect sagging. Where were the original implants placed: behind the breast or behind the muscle?

Over the muscle is a disaster; implants should almost always go under. This is a huge implant to start with and increasing about 1 1/2 cups is going to cause sagging. If you are already over the muscle, it is very difficult to go under and after a couple of years over the muscle, it will be thinned to where you cannot go under. If you are under now, stay that way. Expect capsular contracture from the change out.

I specialize in XL breast implants. Normally, on top of the muscle allows larger sizes to be placed, but under you have more coverage of the implant. You would need to discuss the benefits of each in your particular case with your board-certified plastic surgeon.

Yes, under the muscle is better. However, you may also need a breast lift. Otherwise, your breasts will be very large and potentially sag.

Thank you for your question. This is an excessively large implant that will eventually cause breast sagging and thinning of your breast tissue, especially if it is placed on top of your muscle. If your are on top of the muscle now and want to place the implant under the muscle, you will most likely need to have a breast lift or mastopexy procedure to treat the loose breast skin.

Increasing breast volume can definitely lead to worse sagging, and this really has no bearing on whether the breast implant is placed over or under the muscle. Expect worse sagging on the left, which could be corrected with a periareolar breast lift.

If you have sagging in one breast more than the other, that difference will still appear. In my opinion, it may appear worse if going on top of the muscle.

1050 cc saline implants, above the muscle, will likely have visible and palpable ripples. Also, the skin will stretch over time. Also, the breast mound will ultimately sit low on your chest, necessitating a pocket modification, increasing the work and costs of your next operation.

Probably better to stay above the muscle. The breasts will be perkier with larger implants.

I would be happy to answer this question for you. So, at our facilities, we only go under the muscle. Since your going larger, there won't be sagging. If you were going smaller, then yes there would be some sagging.

Thank you for your question. It is very difficult for online consultants to give you accurate advice without an examination and/or seeing your photos as well as some goal photos.
In general, I prefer to place implants under the muscle for better, longer lasting results.

There is no such implant as a UHP 1050cc. Perhaps you mean overfilling a saline moderate plus or HP implant to hit that volume. It is impossible to say what type of sagging may or may not occur over time with that much volume but, in general, the more breast volume, the higher the chance that the tissues will relax and stretch over time. Also, I would not expect to see a large volume change going from 700cc to 1000cc. In layman's terms, that is less than a can of coke distributed evenly around the breast volume. Most of my patients who are making the jump past the 700-800cc mark usually end up near the 1300-1500cc range when it is all aid and done. I hope this helps.

Initially, the increased volume may take up some of the skin laxity and partially correct the appearance of the sagging, but over the long-term this will likely set you up for more sagging as the weight of the implant acts to stretch out the skin. I am sure that you are aware that this is a much larger implant than average. In fact, there are no implants that are intended to be this large.
