I have had 3 corrective surgeries in the last 3 months because the implants seem to drop thru the pocket. What can be done?

They hurt and look distorted. More permanent stitches were put in last time but is there a permanent fix? I have 595 cc silicone.

It is unfortunate that you have been through so many revision surgeries in such a short time. Perhaps you should consider a second opinion consultation with another plastic surgeon. Permanent is not the best term as the nature of breast implants is never permanent in that they are not life time devices. Understandably, you are looking for a more "long lasting" result. A good option to consider is the use of a support mesh material such as Galaflex. Another consideration is that any type of support repair with sutures and/or mesh needs stable tissue such as the implant capsule to which to affix the support. This tissue does not become strong for at least 3-6 months after surgery and therefore the failure of your repairs may be inevitable without waiting a sufficient period.

You need an Alloderm graft as your tissues will not support the implant.

It may be necessary to add a mesh of some type. This may better ensure that your implants will stay in place.

I make a neo-pocket as well as add an internal bra support that should correct the problem.

It is somewhat unusual to have 3 corrective surgeries in 3 months. At this time, it might be best to allow the tissues to heal. Once things settle down, you might need a technique that utilizes ADM material to properly stabilize the breast implant pocket. Seek out an experienced and skilled surgeon for an in-person consultation.

Without more detailed information about your surgical history and performing an examination, it is difficult to give good advice.
In general, I ask patients to wait for revision surgery because we need the capsule that has formed around the breast implant to be strong enough to hold the permanent sutures that will be used for the capsulorrhaphy procedure. The capsule strengthens with time and so I suggest that patients are 9-10 months post op before we proceed with the internal bra procedure.
Strattice and/or biosynthetic mesh can also be used for additional support.

If your implants continue to bottom out, pushing past the natural fold position, then you need to reinforce and secure the pocket/ fold with some additional tissue - either an acellular dermal matrix like Strattice or a mesh like Seri Silk. You may also need to downsize the implants as well to lighten the overall weight. Once you have failed permanent suture repair, there really are not any other options.

I'm sorry to hear about your multiple operations. Obviously, your surgeon is trying to achieve the best possible result. Depending on your height and weight and tissues, perhaps 595 is too large of an implant for your tissues to support. While other materials such as acellular dermal matrix (Alloderm), Strattice, or Seri may help to provide lower pole support, you may want to consider downsizing if revision is neceasary
