My Dr. wants to do overs 400cc. I am afraid of rippling. Or not enough upper pole fullness. Does anyone have photos of overs? Also, what are the benefits?
Overs will give you more upper pole fullness especially with a high profile implant. Your concerns about rippling are real--if you have enough breast tissue to cover the implant you should be OK. In your lifetime, the breast tissue usually decreases at which time you could see rippling.

I individualize every patient's procedure depending on how they look and what they are trying to achieve. The main benefit from putting the implants under the muscle was that it seemed like the chances of hardness or capsular contracture were less but you still would feel the implant in places--you had more covering of the implant. The negative is that you do get muscle distortion, especially when you contract your chest muscle. I do talk about the positives and negatives with my patients during my consultations, and then we both decide which way to proceed.

In general, I recommend under the muscle breast augmentation as this reduces the chance of capsular contracture and rippling. Every patient's needs are different. Best to discuss in detail with the surgeon whom you have chosen to perform your surgery. Best wishes for a result you will be happy with.

Rippling can be more visible when implants are placed over the muscle, especially if you don't have much breast tissue. Over the muscle does not have the same support as under the muscle, so it is possible you will experience some sag and lack of upper fullness. The weight of the implant could increase these issues over time as well. The benefits of over the muscle are simply an easier recovery, possibility of closer cleavage, and in some cases a more maternal/natural appearance (i.e. because there tends to be some sag).

Overs carry a higher risk of rippling, tissue stretch, sagging, and capsular contracture. If he isn't an ABPS certified/ASAPS member surgeon expert in all forms of breast surgery (not just implants) you need to visit a few that are.

Generally, over the muscle can lead to more rippling depending on the thickness of your tissues. The advantage is less pain with surgery and less motion with muscle activity. The disadvantages, besides rippling, are a higher rate of capsular contracture and less optimal mammogram visualization. Sagging is less dependent on above or below muscle and more related to your baseline breast droopiness.
